Abstract
Learning Outcomes
This case can be used to teach students the design and implementation of integrated care models in chronic disease management within resource-constrained health systems. Upon completion of the case discussion, students will be able to explain the principles of holistic, patient-centred care and assess its impact on patient outcomes, satisfaction, and loyalty. They will evaluate the operational and financial challenges of transitioning from transactional to outcome-focused healthcare models, analyse stakeholder roles in integrated care systems, and explore strategies for implementing and scaling coordinated care programs. Additionally, students will examine real-world issues such as health navigation, multidisciplinary teams, system automation, and the use of education and empowerment in chronic disease control.

The case follows the journey of Joseph*, a patient with diabetes and hypertension, and explores how the integrated care programme impacts his health outcomes and overall experience.
